मृत्यु हे घरी झालेले असून, मृत्यू दिनांकापासून पासून 21 दिवसाच्या आत नाव निशुल्क नोंदविले जाईल. तसेच मृत्यू दिनांकापासून पासून 1 वर्ष्याच्या आत मयताचे नाव नोंदवायचे असल्यास 5 रुपये फी आकारली जाईल.

Free, to register decesed name if death happend in house and reported within 21 days from the date of death. And to register decesed name if death happend in house and reported within 1 year from the date of death 5 Rupees Late fee will be charged.

मृत्यु हे घरी झालेले असून, मृत्यू दिनांकापासून पासून 1 वर्ष्याच्या आत मयताचे नाव नोंदवायचे असल्यास
खाली नमूद केलेले कागदपत्रे आवश्यक आहेत : -
1. अर्जदाराचे आधार कार्ड
2. मयताचे आधार कार्ड
3. स्मशानभूमीचे दहन/दफन केलेल्याचे पावती
4. दैद्याकीय प्रमाणपत्र 4A.
5. मृत्यू वार्ता अहवाल म. न. पा प्रत.

below mentioned documents are mandatory : -
1. Applicant's Aadhar card
2. Dead Perason Aadhar card
3. Cremetion Receipt
4. Medical Certificate 4A.
5. Mrutyu Varta Ahwal
